  • This position is responsible for the post discharge coordination of follow-up care provided to liver, kidney, and pancreas recipients after they have received a transplant.
  • The staff member functions as a liaison between transplant surgeons, nurses, pharmacies, laboratories, home health agencies, and other support services needed.
  • Candidates must possess the ability to assess data reflective of the patient's status and formulate an appropriate course of action in collaboration with the transplant team.
