Job Summary
This role is responsible in assisting in planning, developing, organizing, implementing, evaluating, and directing our facility’s social service programs to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
Assist residents in achieving the highest practicable level of self care, independence and well being, and with providing medically related social services so that the highest practicable physical, mental and psychosocial well being of each resident is attained or maintained.
Maintain the confidentiality of all resident care information including protected health information and report known or suspected incidents of unauthorized disclosure of such information.
