Job Summary

  • The Care Coach provides comprehensive initial and follow-up assessments in client’s home, in accordance with program protocols.
  • Utilizing motivational interviewing, identifies client and family service needs and goals and develops a set of recommendations to meet client/caregiver needs and goals.
  • Contacts all prospective caregiver coaching clients within 24-28 business hours and every 2 business days thereafter to maintain strong pipeline.
