Job Summary

• Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks facility utilization of PDPM tools.
• Ensures accurate and timely completion of all MDS Assessments and coordinates interdisciplinary participation in completing the Minimum Data Set (MDS).
• Responsible for all level of care changes within the facility and maintains the confidentiality of all resident care information.
