Job Summary

  • Provide supportive Health Home care management utilizing the process of care coordination for individuals identified as high users of services.
  • Responsible for the overall creation, management, and coordination of the enrollee’s care plan, encompassing medical/behavioral and social service needs.
  • Caseload will include clients with a mid-acuity rating and voluntary assisted outpatient treatment (AOT) clients, requiring weekly reports in accordance with AOT guidelines.
