Job Summary
Provides comprehensive psychosocial, emotional, spiritual, and bereavement support to Hospice patients and their families.
Conducts assessments, develops and implements goal-directed plans of care, provides counseling and crisis intervention, and coordinates community resources.
Serves as an integral member of the Hospice Interdisciplinary Group (IDG), ensuring care aligns with hospice philosophy, regulatory standards, and individualized patient and family needs.

Key Requirements
LMSW credential
MSW from CSWE-accredited school with 1+ year healthcare experience
BSW from CSWE-accredited school with 1+ year healthcare experience and MSW supervision
Bachelor's degree in psychology, sociology, or related field with 1+ year social work healthcare experience and MSW supervision
Minimum two years social work experience preferred
Active patient contact within past three years preferred
