Job Summary

• The Home Care Coordinator is responsible for the support and implementation of home care services for participants.
• Provide support with the evaluation of a safe home environment through home visit assessments and coordination with home caregiver vendors and family caregivers, coordination of durable medical equipment, and nursing home care.

Key Requirements

• Current CNA certification issued by the State of California
• Minimum of 1 year of experience practicing as a CNA
• Experience working with a frail or elderly population preferred
• Valid driver's License
• Valid BLS certification or higher
