Key Requirements

• Bachelor's degree in Nursing or Physical Therapy
• 2-5 years experience in acute care nursing or therapy
• 2-5 years experience in Home Care or Hospice case management
• 2-5 years experience in Home Care or Hospice compliance or regulatory
• 2-5 years experience in Home Care or Hospice state and federal regulations
• RN or MBN preferred
• Certified Case Manager (CCM) preferred
• American Nurses Credentialing Certification (ANCC) preferred
• OASIS Certification preferred
