case management and discharge planning
psychosocial assessments and interventions
collaborate with multidisciplinary teams
knowledge of Medicaid and Medicare
experience with hospital admissions and discharges
Nice-to-have
passion for mental health support
understanding of Idaho mental health procedures
experience with both adult and pediatric patients
familiarity with community resources
ability to facilitate treatment planning
Key Requirements
Licensed Clinical Professional Counselor or equivalent license
Two years post-license mental health experience preferred
Basic Life Support certification required
Case management and discharge planning experience essential
