Job Summary
Provides clinical social work services to patients and their families in Hospice, Home-Based Palliative Care, and Homecare programs.
Functions as a member of the interdisciplinary team, offering psychosocial evaluations, counseling, grief/bereavement services, and community outreach.
Responsible for Hospice Volunteer and Hospice Bereavement programs, and provides consultation and education to home care staff.

Key Requirements
Master’s degree in social work
Licensed by Ohio Counselor, Social Worker, Marriage and Family Therapist Board
Valid Ohio driver’s license and auto insurance
Pass motor vehicle background inspection
Two years of previous hospice, home health or medical social work experience preferred
Experience working with children and families affected by chronic/life-threatening illness
