Job Summary

  • Monitors, coordinates and evaluates the care and appropriateness of hospitalization; initiates referrals.
  • Completes Continuity of Care assessments and establishes appropriate plans by initiation of timely interventions through collaboration with patients, families and multidisciplinary care team respecting choice of patient.
  • Registry employees who meet eligibility may participate in one of our 401(k) Savings plan with a potential match.

Key Requirements

  • Currently licensed as a Registered Nurse in the state of Illinois
  • BSN preferred
  • 2-5 years previous Case Management experience preferred
