  • Perform an initial examination and evaluation to establish an impairment/diagnosis, prognosis, and plan of care prior to intervention for all persons served with basic and some complex rehabilitation needs.
  • Communicate, coordinate, and document all aspects of patient/client management including the results of the initial examination and evaluation, diagnosis, prognosis, plan of care, interventions, response to interventions, barriers to progress, changes in patient/client status relative to the interventions, reexamination, and discharge/discontinuation of intervention and other patient/client management activities with members of the care team as indicated.
