Job Summary
WelbeHealth PACE provides seniors with the opportunity to continue living in their homes and in their communities.
The Primary Care Provider works collaboratively with WelbeHealth Physicians and the IDT to complete assessments, plan primary care, and educate WelbeHealth participants and their families.
