Job Summary
This job assists in data collection and implementation of the delivery of health care services, assisting other members of the health care team in providing the highest quality personalized patient care.
Engages the patient to maintain optimal health and reduce or minimize acute exacerbations associated with chronic conditions that affect quality outcomes and costs.
Remains knowledgeable on current federal, state and local laws, accreditation standards or regulatory agency requirements that apply to the assigned area of responsibility and ensures compliance with all such laws, regulations and standards.
