Key Requirements

  • MD or DO degree from accredited school
  • ACGME accredited residency in Family Medicine
  • ABMS board certification or eligibility
  • Active or ability to obtain Utah medical license
  • BLS certification and DEA license
  • Completion of Intermountain Health credentialing process
  • No Visa sponsorship available
