Job Summary

  • Oversee the facility to ensure assessments are completed per guidelines, with a primary focus on the resident assessment process, including individual assessment, MDS assessment, and resident plan of care.
  • Track and monitor facility effective utilization of PDPM tools and performance of QM and QASP, providing data analysis and clinical recommendations for optimum outcomes.
  • Coordinate interdisciplinary participation in completing the Minimum Data Set (MDS) for each resident according to regulatory time frames, ensuring completeness and thoroughness of documentation.
