Job Summary
Provides clinical assessment, behavioral health treatment, case management, care coordination, and crisis intervention for children, adolescents and their families across all professional and clinical settings.
Assists in developing specific, realistic, and measurable treatment goals and objectives, and accurately assesses and documents client presenting problems, history, symptomology, mental status, and diagnosis.
Coordinates client care with other service providers and community resources to assure continuity of care and completes clinical charting and other related administrative paperwork in accordance with policy standards.
