Thyme Care is seeking a Comprehensive Health Assessment Nurse Practitioner to provide virtual care and support for cancer patients. The ideal candidate will have at least three years of NP experience, including oncology and telehealth, and a passion for compassionate patient care

Job Summary

  • Conduct the first and annual clinical interaction with members using a telemedicine platform for a thorough, whole-person, comprehensive history and exam.
  • Evaluate risk scores, assess gaps in care, and develop comprehensive care plans to address clinical and non-clinical needs and barriers, prioritizing member goals.
  • Review medical records, work with external providers, and provide clinical insights to the care team while adhering to best documentation practices to drive high-quality care.
