Job Summary

  • Completes psychosocial assessments for patients' post-hospital care and their support systems, assisting them in coping with stress related to illness and hospitalization.
  • Works with the Clinical Care Management team to assess and evaluate patients for appropriate care levels and serves as a resource to hospital staff and physicians on psychosocial components of illness.
  • Provides information and education on community resources and post-hospital care options, making referrals as needed and reporting suspected cases of abuse/neglect/exploitation.
