  • Must be able to effectively communicate with, and promote cooperation and collaboration between individuals including residents/families/caretakers, physicians, nurse and other ancillary partners.

Key Requirements

  • Bachelor’s Degree in Nursing or Social Work
  • Registered Nurse (RN) license preferred
  • Licensed LVN or LPN acceptable
  • Two years clinical nursing experience preferred
  • Knowledge of Medicare, Medical and Medicaid programs
  • Valid LVN/LPN or RN license in state of work
  • Certified Case Manager (CCM) certificate a plus
  • Maintain required continuing education and licensing
