Must be able to effectively communicate with, and promote cooperation and collaboration between individuals including residents/families/caretakers, physicians, nurse and other ancillary partners.

Key Requirements
Bachelor’s Degree in Nursing or Social Work
Registered Nurse (RN) license preferred
Licensed LVN or LPN acceptable
Two years clinical nursing experience preferred
Knowledge of Medicare, Medical and Medicaid programs
Valid LVN/LPN or RN license in state of work
Certified Case Manager (CCM) certificate a plus
Maintain required continuing education and licensing
