• The Health and Wellbeing Coach provides personalised support to individuals to empower them take control of their own health and wellbeing with a focus on lifestyle behaviour change.
• You will facilitate people in establishing and attaining health-promoting goals, set by the person to change lifestyle-related behaviours, with the intent of reducing health risks, improving self-management of chronic conditions, and improving quality of life.
