Job Summary

  • Assists clients by assessing needs for available support services and facilitates access to program and State plan and other services.
  • Develops Individualized Service Plans (ISPs) for assigned participants based on assessment of needs and using person-centered planning process.
  • Conducts in-home/on-site visits to participants to assess quality of care and monitor services and supports received.
