Job Summary

• Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks PDPM tools utilization.
• Assesses and determines the health status and level of care of all new admissions, ensuring accurate and timely completion of all MDS Assessments.
• Responsible for all level of care changes within the facility and maintains the confidentiality of all resident care information.
