Otterbein is seeking a Home Health & Hospice Social Worker to provide psychosocial support for terminally ill patients and their families. The ideal candidate will possess a Master's degree in social work, an active Ohio social work license, and at least one year of experience in a healthcare setting, preferably in hospice or home care

Job Summary

  • The Home Health & Hospice Social Worker is responsible for the provision of a wide range of psychosocial services to terminally ill patients and their families.
  • Conduct initial comprehensive psychosocial and bereavement risk assessment of patient/family/caregiver within five (5) calendar days after the patient’s election of Hospice care.
  • Participate in the development of the plan of care and treatment and attends and participates in scheduled interdisciplinary team meetings (IDT) to coordinate care plans.
