Job Summary

  • Establishes course of action by exploring options and setting goals with residents and their families to assist them in achieving the highest practicable level of self-care, independence, and well-being.
  • Provides medically related social services to ensure the highest practicable physical, mental, and psychosocial well-being of each resident is attained or maintained.
  • Develops and maintains strong working relationships with other departments and outside community agencies to meet resident needs, including coordinating discharge planning and in-service training.
