Job Summary
Establishes course of action by exploring options and setting goals with residents and their families to assist them in achieving the highest practicable level of self-care, independence, and well-being.
Provides medically related social services to ensure the highest practicable physical, mental, and psychosocial well-being of each resident is attained or maintained.
Develops and maintains strong working relationships with other departments and outside community agencies to meet resident needs, including coordinating discharge planning and in-service training.
