Job Summary

  • The Care Partners assist in providing services tailored to the needs of individual clients, delivered within budget and service expectations, including ongoing client assessment and the development of care plans in consultations with key stakeholders.
  • Provide professional and competent day-to-day delivery of services to clients in line with service expectations and work to ensure that staff have a respectable understanding of our individual clients’ needs and preferences.
  • As a not-for-profit, Christian organisation, we provide expert aged care, home care, dementia care and palliative care.
