Job Summary
The RN will provide patients who have chronic conditions the support, the education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state.

Key Requirements
Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing
Valid RN License
Minimum of 1 year Registered Nursing experience
Baccalaureate Degree, ADN, or Diploma RN Degree
3 years RN experience
2 year experience in a clinical environment
Current BLS certification
ACLS certification or obtain within one year
Knowledge or experience in PCMH
Supervisory experience
Proof of legal authority to work in the United States
