Job Summary

  • As a Clinical Pharmacist, you will play a vital role in ensuring patients receive accurate, timely, and respectful medication care—selecting, dosing and consulting on treatments that improve health and well-being.
  • In this unique role, you will support patients who are considered inpatient but reside at home, reviewing charts, verifying medications and collaborating closely with physicians on care transitions.

Key Requirements

  • PharmD or Bachelor’s of Science in Pharmacy
  • Current state licensure as a Registered Pharmacist
  • Current Basic Life Support (BLS) Certification
  • Current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certifications
  • Board Certification
  • Completion of a Post-Graduate Year 1 Residency (PGY1)
