The Senior Population Health Coordinator leads population‑level initiatives to improve health outcomes for individuals and families served by the Healthy Community Center in Columbus, Ohio.
The Senior Population Health Coordinator is also responsible for population health data collection and analysis, developing and maintaining dashboards to track patient outcomes and program metrics, and conducting comprehensive program evaluations to assess effectiveness and inform continuous quality improvement.
