Circleville Post Acute is seeking a full-time MDS Assistant (RN) to oversee the completion of MDS assessments, coordinate interdisciplinary participation, and ensure compliance with federal and state regulations. The ideal candidate will possess a Bachelor of Science in Nursing, a minimum of three years of supervisory experience, and strong organizational and analytical skills

Job Summary

  • Provides oversight on facilities’ weekly PDPM meetings for skilled patients.
  • Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change.
  • Responsible for all level of care changes within the facility.
