Job Summary
Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks facility effective utilization of PDPM tools.
Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change, and coordinates interdisciplinary participation.
Responsible for all level of care changes within the facility, notifying all departments and transmitting forms to the appropriate agency for processing.
