As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers

Job Summary

  • As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.
  • Initiate communication with attending physicians, Hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient and loved ones/caregivers.

Key Requirements

  • Graduate from an accredited School of Nursing
  • Currently licensed RN in Georgia
  • One year of RN experience
  • CPR certified
  • Licensed driver with insured automobile
