Pharmacy medication reconciliation technician is a certified pharmacy technician who supports pharmacy services by obtaining medication and allergy histories for patients that will be admitted to the hospital.
Information is obtained by interviewing the patient, patient family, calling pharmacies, long term care facilities and/or other providers.
