Job Summary
Establishes course of action by exploring options and setting goals with residents and their families to assist them in achieving the highest practicable level of self-care, independence, and well-being.
Provides medically related social services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident, including emotional support and grief counseling.
Develops and maintains strong working relationships with internal departments and external community agencies to meet resident needs, including coordinating with interdisciplinary teams and assisting with discharge planning.
