Forest Grove Post Acute is seeking a full-time Case Manager - Infection Preventionist who utilizes clinical expertise to develop and implement care plans for residents. The ideal candidate will have a Bachelor’s Degree in Nursing or Social Work, with a preference for those holding a Registered Nurse license, and will work collaboratively with various stakeholders to ensure high-quality patient care

Key Requirements
Bachelor’s Degree in Nursing or Social Work
Registered Nurse (RN) license preferred
Licensed (LVN or LPN) nurse acceptable
Two (2) years of clinical nursing experience
Knowledge of Medicare, Medi-cal and Medicaid
Certificate as a certified Case Manager (CCM) a plus
