Job Summary
We are St Vincent’s Health Australia, Australia’s largest not-for-profit provider of health and aged care services.
Using a case management model and in collaboration with the CNS for HIV complex care and the HIV social work team, provide support and care for patients living with HIV with complex bio-psychosocial needs promoting medication adherence, engagement in care, harm minimization and service(s) navigation.
