Job Summary

• The primary purpose of this position is to assist in planning and directing social service programs to meet the medically related emotional and social needs of residents.
• Employees will coordinate social service activities with other departments and assist in obtaining resources from community agencies to support resident well-being.
• The role requires maintaining strict confidentiality of all resident care information including protected health information and reporting any unauthorized disclosures.
