The Advanced Practice Provider collaborates with physicians and other clinicians to provide a full scope of patient care focused on preventive health and management of conditions.
The APP provides care during acute and chronic phases of illness, including education, counseling, health promotion, and referrals to other healthcare providers and community resources.
