Job Summary

  • The Health Services Coordinator serves on the health services team supporting the overall operations of the department through a variety of administrative and clinical functions as directed by the Director of Health Services (DHS).
  • Conduct and coordinate assessments / evaluations of potential residents and make recommendations for admission in accordance with current rules, regulations, and community policies and procedures that govern resident assessment.

Key Requirements

  • RN, LVN or LPN License
  • 2+ years of prior related work experience

  • Ability to lift and carry up to 50 pounds
  • Ability to assist in the physical movement of residents
