Job Summary
The role systematically intervenes to provide clinical social work and complex discharge planning to patients and families with psychosocial needs.
This position requires participation in an interdisciplinary team to ensure psychosocial issues are treated across the continuum of care for high-risk populations.
The successful candidate will develop treatment plans using clinical diagnoses and access community resources to meet identified patient needs.
