PAHrtners Deaf Services is seeking a part-time CAP Case Manager for the Weaverville - Mars Hill area to provide essential case management services for medically fragile children. The role requires a Bachelor’s degree in a human services field and experience in case management, focusing on supporting individuals with intellectual and developmental disabilities

Job Summary

  • Provide basic case management services to medically fragile children who are at risk of institutionalization and whose health, safety, and well-being can be maintained in his/her primary residence.
  • The Community Alternative Program for Children is a Community Based Services Waiver designed to provide an alternative placement in a nursing facility for beneficiaries who prefer to remain in his or her primary private residences and would be at risk of institutionalization without these services.
