The RN Case Manager is responsible for assessing patient needs, coordinating the Plan of Care with an Interdisciplinary Team, and providing clinical palliative care to keep participants in their homes.
Employees must be able to perform physical requirements including lifting patients, bending, and visiting participant homes while maintaining confidentiality and compliance with regulations.

Base: $75,895.65 - $113,843.48; Bonus: $10,000 Sign On Bonus; Benefits: Start on Day 1
Skills & Requirements
Must-have
Current RN license in state of work
Minimum two years pediatric nursing experience
Valid driver's license and reliable transportation
Active BLS certification from AHA or Red Cross
Experience with EMR/EHR systems
Nice-to-have
Hospice or hospital experience preferred
Ability to communicate empathetically with families
Strong relationship building with physicians
Experience with interdisciplinary team collaboration
