Identify and navigate patient testing and treatment to reduce barriers to patient discharge and preventing delays in patient care; communicates barriers to leadership for resolution and trending.

Key Requirements
Registered Nurse (RN) or Social Worker (SW)
RN: Associate of Science degree required, Bachelor’s degree preferred
RN: Minimum of 1-3 years clinical experience
RN: Licensed as Registered Nurse with Maryland Board of Nursing
SW: Bachelor of Social Work (BSW) or Master of Social Work (MSW)
SW: Minimum of 3 years of experience in diverse clinical setting
SW: LBSW, LMSW, LCSW
Active American Heart Association Basic Life Support (BLS) certification
