Job Summary

  • Responsible for the identification, resolution, and prevention of medication related issues, providing drug information and recommendations for optimization of medication therapy to physicians, nurses and patients.
  • Develops competency in clinical practice including appropriate drug therapy for patients and prevention/resolution of drug related problems, individualizing drug therapy regimens based on patient’s size, organ functions, and disease states.
