Job Summary
Responsible for the identification, resolution, and prevention of medication related issues, providing drug information and recommendations for optimization of medication therapy to physicians, nurses and patients.
Develops competency in clinical practice including appropriate drug therapy for patients and prevention/resolution of drug related problems, individualizing drug therapy regimens based on patient’s size, organ functions, and disease states.
