  • Responsibilities include conducting initial assessments, maintaining patient eligibility records, managing intake and enrollment, and making timely referrals to Part B and other supportive services to ensure continuity of care.
  • The coordinator plays a key role in care coordination, patient education, and eligibility management, working closely with clinical staff and regional case management teams across Central Kentucky and Southern Indiana.
