Job Summary
Comprehensive clinical assessment, requesting diagnostic investigations, management of care for patients/clients, prescribing medicines/therapies, including direct referral of patients to and from other health care professionals.
Using a case management model and in collaboration with the CNS for HIV complex care and the HIV social work team, provide support and care for patients living with HIV with complex bio-psychosocial needs promoting medication adherence, engagement in care, harm minimization and service(s) navigation.
