Job Summary

  • The HF APC is responsible to facilitate the care and education of HF patients and their families from initial contact and throughout their continuum of heart failure care.
  • Perform assessments of patients within the clinic to establish, implement and evaluate individualized plan of care.

Key Requirements

  • 3-5 years of clinical experience in an acute care setting
  • Graduate of an accredited PA program or MSN/BSN with Master's in advanced practice nursing
  • Unrestricted PA or NP license in Virginia
  • Unrestricted DEA license in Virginia
  • PA board certified by NCCPA or NP board certified (FNP, AGPCNP, AGC-ACNP)
  • AHFN Certification required or obtain within 24 months of hire
  • AHA BLS Certification within 30 days of hire
  • AHA ACLS Certification within 90 days of hire
