This position involves two components: providing care coordination and physiotherapy in the community, and within the Emergency Department.
In the community, you will oversee a small caseload of clients with varied health and social needs in a person-centered approach.
ED care coordination will include comprehensive assessment of patients within identified target groups to ensure safe discharges and referrals to appropriate services.
