Centerville Post Acute is seeking a full-time MDS Assistant (LVN) to oversee weekly PDPM meetings, monitor performance metrics, and ensure compliance with MDS assessments. The ideal candidate will have a nursing background with supervisory experience, particularly in long-term care settings

Job Summary

  • Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks and monitors facility effective utilization of PDPM tools.
  • Tracks and monitors QM and QASP performance monthly and quarterly and provides data analysis and clinical recommendations for facility for optimum outcomes.
  • Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change, and coordinates interdisciplinary participation in completing the Minimum Data Set (MDS).

Key Requirements

  • Proficient in Microsoft Suite products
  • Knowledge and experience with PCC preferred
