Key Requirements

• Bachelor's Degree in Pharmacy or PharmD
• PGY-1 AND PGY-2 Pharmacy Residency or equivalent experience
• Oregon Pharmacist License upon hire
• Board of Pharmacy Specialties Certification within 2 years
• Two years Senior Ambulatory Clinical Pharmacist experience OR Five years pharmacy practice with 2 years relevant experience
